				<content:encoded><![CDATA[<p><P><BR><BR><B>The Norwood Scale Measures <span class='bm_keywordlink'><a href="http://www.malepatternbaldnessblog.com">male pattern baldness</a></span></B><BR><BR><B>The Norwood Scale illustrates the progression from normal hair, through the thinning process, to balding in men.</B><BR><BR>The Norwood Scale (sometimes called the Norwood-Hamilton Classification, or Scale) is a graphic representation of the progression and patterns that <B>male pattern baldness</B> takes over time. The companion Ludwig Scale and Savin Scale, the latter of which also measures overall thinning, represent the typical <span class='bm_keywordlink'><a href="http://www.malepatternbaldnessblog.com/go/hairlossblueprint2/" rel="nofollow" target="_blank">hair loss</a></span> paths that are experienced by women who have progressive female pattern baldness. All are ways to predict and discuss degrees of hair loss due to androgenic (also called androgenetic) alopecia.<BR><BR>On the Norwood Scale, classes 1 through 4 show a progression from &#8220;normal&#8221; (preloss) through recession at the temples, the frontal hairline and the crown. A person might say of an individual in these categories, &#8220;His hair is thinning.&#8221; Classes 5 through 7 illustrate the more significant loss categories, where one would more likely be called &#8220;balding&#8221; or &#8220;bald.&#8221;<BR><BR>The Ludwig Scale is somewhat different, just as the patterns of hair loss in women differ from those in men. Female baldness tends to happen in a more generalized way, although a complete loss at the crown of the head is not uncommon. (Two additional measures, the Olsen Scale and Ebling-Rock Scale, are used to a lesser extent to measure female hair loss.)<BR><BR><B>How the Norwood and Ludwig scales are used</B><BR><BR>Hairstylists and <B>hair transplant</B> surgeons use both the Norwood and Ludwig scales to educate their clients and patients, respectively, on the likely progression their hair loss would follow over time. The scales are not useful for discussing nonprogressive hair loss (e.g., from chemotherapy or temporary hormonal disruptions, such as the hair loss experienced by women who are pregnant or postpartum).<BR><BR>&#8220;It&#8217;s a common language when talking to a patient,&#8221; explains hair transplant surgeon Dr. Robert Leonard, whose Leonard Hair Transplant Associates works out of Boston and Hyannis (Cape Cod) and in New Hampshire and Rhode Island. &#8220;I show patients where their hair loss will go if they delay any form of treatment. As with other cosmetic surgeons who perform hair restoration transplant surgery, Leonard encourages patients to use multiple therapies to halt hair loss. These can include using surgery and a medication, such as minoxidil or finasteride, in combination.<BR><BR>Another hair transplant surgeon, Dr. Craig Ziering, whose Ziering Medical Worldwide offices are located in southern California (Beverly Hills and Newport Beach) as well as Chicago, Las Vegas and Salt Lake City, half agrees with Leonard. &#8220;It helps educate patients,&#8221; he says, &#8220;but a lot of people fall outside of it,&#8221; simply because of genetic differences.<BR><BR><B>The Norwood Scale &#8212; applicable only to Caucasians?</B><BR><BR>Based on most depictions of the Norwood Scale, it might appear to be racially limited, applicable only to white males. Dr. James Hamilton after all, initially developed it in the 1950s. Dr. O&#8217;Tar Norwood updated it in the 1970s. The features of the various drawings, profiles and the tops of heads typically appear to be Caucasian.<BR><BR>&#8220;It does not apply only to Caucasians,&#8221; says Dr. Jonathan Ballon, a physician and neurosurgeon who has performed more than 2,500 hair transplant procedures. Ballon works with Hair Club in its Atlanta, Charlotte (North Carolina), Chesapeake (Virginia), Columbia (South Carolina), Knoxville (Tennessee), Raleigh (North Carolina) and Tysons Corner (Virginia) locations. &#8220;The Norwood Scale is an imperfect attempt to categorize male pattern baldness into neat little boxes. Sometimes it is helpful, but often it is not.&#8221; Hair Club and other transplant surgical centers feature &#8220;before&#8221; and &#8220;after&#8221; photos of people of various races &#8212; and genders.<BR><BR>Hair loss differences between ethnic groups, &#8220;are more often seen in women,&#8221; says Dr. Leonard. &#8220;Their loss is more generalized, not the M-shaped recession more typically found in men.</P></p>

The advanced technology is required in this method in order to help the patient get their confidence back with the growth of the hair. The method of FUE hair transplant is usually called follicular unit extraction. The major concept of the method is harvesting hair from certain donor site. The experts use precision tools to apply the method. Hair on certain area is harvested for the balding areas. The idea of the method seems simple but it requires advanced technology to ensure the effect. </P><br />
<P>It is mainly the extraction process, the manner in which the hairs are harvested, that differs between the two major techniques. The FUE (Follicular Unit Extraction) method almost always takes longer than the Strip, unless there is a very small area that is receiving the hair transplant. The reason why the FUE takes longer is because the extraction process is difficult and requires a very high degree of skill and precision. During the FUE, the doctor will extract each hair follicle manually, by hand. Since each single follicle is extracted individually this is a time consuming procedure. The benefit is that controlled and careful extraction of donor hair is very soft to the skin and scalp. It is very hard to notice for scars even after only a few weeks. Complications are very rare. </P><br />
<P>FUE hair transplant surgery is better and comfortable as compared to the older hair transplantation procedures that used larger grafts and often produced a plugged, unnatural look. </P><br />
<P><STRONG>How a FUE Hair Transplant Works</STRONG><BR>The follicular unit extraction is removed using a multiple step process. During the first step, also called FUE (Follicular Unit Extraction), direct extraction of follicular units from the patient&#8217;s donor area is performed. It allows the surgeon to select individual follicular units from a donor region in a pattern that virtually solves the potential problem of visibly lower donor area density after the procedure. The surgeon uses a small punch (a sort of a special needle) (0.75 &#8211; 0.9 mm diameter) to pluck out the follicular units, eliminating the need for removal of skin from the back of the head. Benefits of FUE are: a quicker healing time, far less trauma, no strip scar, and the surgeon&#8217;s ability to individually select follicular units. Discomfort in the donor area has been practically eliminated.</P><br />
<P>As the route and angle of the follicular unit under the skin can&#8217;t be seen and can often vary from the direction of the hair on the surface, a sharp punch if used below the surface of the skin might transect or severe the underlying follicular unit. However, the dull punch tends to cover the follicular unit, while separating it from the surrounding soft tissues. This process is typically referred to as &#8220;blunt dissection&#8221;.<BR><BR>After the follicle is extracted a small hole is left behind which heals over the next few days. Later on after some time this tiny wound contracts as it heals making the resulting round scar smaller and less noticeable. The FUE patient typically ends up with hundreds of small round white scars, which are normally not detectable to the naked eye once the patient&#8217;s hair grows out. </P><br />
<P>To puncture the sites the surgeon uses a fine needle for implanting the grafts, creating micro-holes (incisions) with predetermined density and pattern, and angling them in an even style to promote a rational hair pattern. This is then followed by the final phase when the surgeon inserts the grafts in the prepared incisions.</P><br />
<P>A skilled and experienced doctor can transplant 1000 to 1200 follicular unit grafts in a day. The cost per graft of FUE is also typically twice the cost of the standard follicular unit hair transplant procedure in which a strip of donor tissue is removed from the back of the head and trimmed under magnification into individual follicular unit grafts. <BR></P></p>

But 14 years later, have real experiences with the product lived up to hype?<BR><BR>Not for everyone. It works for only about 50 percent of patients who try it. Generally speaking, they are under the age of 40 and in an early phase of <span class='bm_keywordlink'><a href="http://www.malepatternbaldnessblog.com/go/hairlossblueprint2/" rel="nofollow" target="_blank">hair loss</a></span> (i.e., thinning began less than five years ago). In other words, start using it as soon as you begin to notice a bald spot.<BR><BR>Note: the second pharmaceutical product available for hair loss treatment, <B>Propecia</B> (finasteride), is reviewed in separate articles on HairLoss.com.<BR><BR><B>Minoxidil works, but only to a certain extent</B><BR><BR>Minoxidil (brand names include Rogaine, Women&#8217;s Rogaine, Rogaine for Men Extra Strength, Regaine, Apo-Gain, Gen-Minoxidol, Hairgro, Minox, Med Minoxidil, Hair Regrowth Treatment) works only on individuals, male and female, with androgenic alopecia (also known as <span class='bm_keywordlink'><a href="http://www.malepatternbaldnessblog.com">male pattern baldness</a></span>). For reasons unknown, minoxidil is slightly more effective for women than for men.<BR><BR>The location of where you are losing your hair seems to matter a lot as well. The crown of the head, the vertex, is much more responsive to minoxidil than is frontal or hairline loss. One study (D. S. Walsh, C. L. Dunn and W. D. James, &#8220;Improvement in Androgenetic Alopecia (Stage V) Using Topical Minoxidil in a Retinoid Vehicle and Oral Finasteride&#8221; [Arch Dermatol, 1995]) found that 36 percent of users experienced sufficient results over 30 months of use such that they felt compelled to spend time and money on the product. Of study participants, 32 percent said new hair growth was long enough that it had to be cut (often the regrowth is more of a peach fuzz than strands of hair).<BR><BR>Not so surprisingly, formula strength matters &#8212; it&#8217;s a situation where more is more. Regular Rogaine, a 2 percent concentration formula, and Rogaine for Men Extra Strength, a 5 percent solution, have been compared side by side in a couple of studies. One (V. H. Price and E. Menefee, &#8220;Quantitative Estimation of Hair Growth: Comparative Changes in Weight and Hair Count with 5 percent and 2 percent Minoxidil, Placebo, and No Treatment&#8221; [Elsevier, 1996]) compared hair mass, which is the weight of hair in a defined area of the scalp, in both products to find that the extra-strength formula indeed produced a 55 percent increase, compared with a 25 percent increase in the lower-concentration formula, after five months of use. Importantly, that mass-effect declined with both products after five years&#8217; use, to 25 percent and 15 percent increases, respectively.<BR><BR>The second study looked at actual hair counts, finding that the higher-strength formula does result in more hair shafts per square centimeter: 25 additional hair follicles per square centimeter from the 5 percent solution, versus 21 follicles in the 2 percent solution and 9 follicles in a placebo group, after a 48-week period (R. J. Trancik, &#8220;Update on Topical Minoxidil in Hair Loss&#8221; [Proceeding of the annual meeting of the American Academy of Dermatology, 1998]).<BR><BR>One effective application is for hair loss due to chemotherapy. Research sponsored by Rogaine&#8217;s manufacturer, the Upjohn Company (now Pharmacia), on 22 women undergoing treatment for breast cancer found that the 2 percent topical solution decreased the duration of alopecia caused by chemotherapy, with no significant side effects (Duvic, Lemak et al., Division of Medicine, University of Texas, M.D. Anderson Cancer Center, 1996).<BR><BR><B>How does minoxidil work?</B><BR><BR>How minoxidil reduces hair loss is actually a mystery, since it was discovered by accident when use of the anti-hypertension drug Loniten was found to increase and darken fine body hairs. One imagines the scientists working on this had their eureka moment within one or two nanoseconds of coming upon this fact.<BR><BR>But two decades later, no one knows for sure how it works. It may be due to the way the medication dilates blood vessels, which on the scalp might stimulate hair growth. Minoxidil is a potassium channel opener, which causes hyperpolarization of cell membranes, possibly another reason it works.<BR><BR>To stem hair loss, minoxidil is applied topically (as a cream or foam). In pill form the medication is in a higher concentration and only approved for treating hypertension.<BR><BR><B>There are side effects to minoxidil</B><BR><BR>For the person first using minoxidil, there is often an initial period of paradoxical increased hair loss. Disturbing as that may be, this seems to be the scalp&#8217;s way of ridding itself of weaker hair follicles to make way for stronger shafts. Other known side effects and issues are:<BR><BR>Scalp irritation, likely because of the product&#8217;s alcohol content<BR>Itchy scalp and dandruff<BR>Dizziness or rapid heartbeat (rare)<BR>Not recommended when the scalp is already irritated, injured or sunburned, which would allow the medication to be absorbed by the body at a higher and potentially dangerous level <BR>Formulated for scalp application and not recommended for use elsewhere on the body<BR>Effects on a fetus in women who are pregnant are unknown; however, women who are nursing are warned to not use it<BR>Rogaine users with severe, refractory high blood pressure might experience hypertrichosis, which is hair growth on the face or other bodily areas. The incidence of this occurs in about 3-5 percent of women who use the 2 percent solution and higher among women using the 5 percent solution.<BR><BR>Note that no animal or human studies indicate cause for concern regarding cancer and minoxidil.<BR><BR><B>Costs of minoxidil</B><BR><BR>According to online stores selling Rogaine, a three-month supply of the foam product is about $50, which is much lower than when the medication was first released (and protected by its original manufacturer by its patent, which expired in 1996).<BR><BR>But at least one company that operated clinics and treatment centers for hair loss, Avacor (Global Vision Product, Inc.), falsely claimed superior treatments that amounted to nothing more than a minoxidil solution repackaged as a shampoo (sold alongside herbal supplements that lacked clinical studies to support claims of efficacy). A class action lawsuit was successful in driving the clinics out of business. This case serves as a reminder that any medical solutions should be investigated closely for legitimacy &#8212; and how hair regrowth charlatans are always looking to make money from people who are experiencing hair loss.<BR><BR><B>Minoxidil: The bottom line</B><BR><BR>To summarize, minoxidil will be most effective if you<BR><BR>Use it as soon as you start to notice a thinning <BR>Are under age 40 <BR>Are a woman; women get better results than men <BR>Use the stronger solution, as it will yield more results <BR>Are attentive to potential side effects, particularly if your scalp is irritated by sun or the medication itself <BR>Stay out of the sun or always wear a hat when outdoors<BR>Accept the fact the results in five years won&#8217;t be as good as in one year<BR>Plan to continue using it to realize any beneficial effect</P></p>

				<content:encoded><![CDATA[<p><P><B>Laser Hair Therapy Couldn&#8217;t Possibly Hurt You &#8212; Or Could It?</B><BR><BR><B>Laser hair therapy has been in use in Europe for decades but the FDA only recently vouched for this <span class='bm_keywordlink'><a href="http://www.malepatternbaldnessblog.com/go/hairlossblueprint2/" rel="nofollow" target="_blank">hair loss</a></span> treatment&#8217;s safety.</B><BR><BR>The application of low-level laser therapy (LLLT), or <B>Laser hair therapy</B>, to address hair loss was first discovered by accident &#8212; with laboratory mice. Researchers were testing laser on wounds, where the hair around the wound was shaved. What they noticed in the mice receiving LLLT versus those who weren&#8217;t (the control group) was their hair grew back faster.<BR><BR>Since then, and for more than 30 years, researchers and hair restoration therapists have been using the technique on humans. Popular for decades throughout Europe, laser hair therapy received approval as safe by the U.S. Food and Drug Administration in 2007. This approval is all about safety &#8212; the FDA does not confirm or deny efficacy.<BR><BR><B>How laser hair therapy safely restores hair growth</B><BR><BR>To appreciate how LLLT is safe, it helps to understand how it works.<BR><BR>LLLT approaches the treatment of <span class='bm_keywordlink'><a href="http://www.malepatternbaldnessblog.com/go/hairlossblueprint2/" rel="nofollow" target="_blank">thinning hair</a></span> from a noninvasive standpoint. Equipment varies somewhat, but between 110 and 116 laser diodes are directed at the scalp, typically applied in a head cap (it can also be administered with a handheld comb). The light is in the 630- to 670-nanometer range, which is where the color red occurs on the light spectrum. The power of the light is at low wattage, which allows it to remain cool throughout the procedure.<BR><BR>The exposure of the scalp to these tiny beams of light is called photobiology, which causes reactions on both physical and chemical levels. Hair follicles, in particular the very tiny hairs on follicles where loss is progressing, absorb this light. The reaction happens on an intracellular level, where cytochrome c, an enzyme, sends photochemical signals throughout the hair follicle. The result is a decrease in apoptosis, or programmed cell death.<BR><BR>The actual efficacy (&#8220;does it work?&#8221;) of laser hair therapy is subject to variations among individuals. Those variations are:<BR><BR><B>What type of hair loss is occurring?</B> Primarily male and female patients with androgenetic (also called androgenic) alopecia can benefit from LLLT. That means it can benefit <span class='bm_keywordlink'><a href="http://www.malepatternbaldnessblog.com">male pattern baldness</a></span> and female pattern baldness, but other causes such as alopecia universalis or trichotillomania will not be affected by it. Some claim it can be used for alopecia areata and with individuals undergoing chemotherapy.<BR><BR><B>Is the hair loss recent or long-standing?</B> Laser hair therapy is probably most effective where hair is actively receding, not on hair lost a long time ago. Men with <span class='bm_keywordlink'><a href="http://www.malepatternbaldnessblog.com">male pattern baldness</a></span> have higher levels of a molecule called prostaglandin D2 in the bald parts of their scalps than in parts still covered in hair, a new study shows.</p>
<p> Prostaglandin D2 stops the growth of stem cells that produce hair follicles, George Cotsarelis of the Perelman Center for Advanced Medicine at the University of Pennsylvania in Philadelphia and colleagues report in the March 21 Science Translational Medicine.</p>
<p> Cotsarelis&#8217; group had previously found that bald men still have hair follicle stem cells, but that those cells are dormant in bald areas of the head. The researchers reasoned that either the stem cells lacked growth stimuli or that an inhibitor prevented the cells from growing.</p>
<p> To find out which, the team analyzed gene activity in scalp samples taken from men undergoing hair transplants. The researchers found 81 genes with higher activity in bald portions of the scalp compared with hair-covered areas. Among the more active genes was one that makes prostaglandin D2.</p>
<p> Other scientists had previously found that a synthetic version of a different prostaglandin called F2alpha stimulates growth of eyelashes. &#8220;Prostaglandins often have a yin and a yang,&#8221; Cotsarelis says: One prostaglandin may stimulate hair growth, but another might stop it.</p>
<p> In the new study, prostaglandin D2 inhibited hair growth in human hair follicles in the lab and slowed hair growth in mice when applied to their skin. And mice genetically engineered to make a lot of prostaglandin D2 in the skin go bald.</p>
<p> Prostaglandin D2 works in the stem cells through a protein called GPR44. That protein, a receptor, sets off a biochemical chain reaction when it detects the presence of prostaglandin D2. Hair growth in mice that lack the receptor wasn&#8217;t inhibited by prostaglandin D2, suggesting that drugs that block GPR44 might help treat baldness, the researchers say.</p>
<p> That doesn&#8217;t mean a cure for baldness is right around the corner, says Kurt Stenn, a hair biologist at the Aderans Research Institute in Marietta, Ga. &#8220;Bald people will have to be patient a little longer,&#8221; he says. The study raises many questions, including why levels of prostaglandin D2 increase in the first place, how the molecule interacts with testosterone (which is known to be necessary for male pattern baldness), and which other proteins might be involved in the balding process, Stenn says. &#8220;It&#8217;s a beginning study, but a wonderful beginning study.&#8221;</p>
